Two lenses, one plan

Earth 土 is the phase it is most often read through. Here the classical lens leads: differential pattern diagnosis decides the treatment, with the Western picture as context.

The Western lensAnatomy, physiology, biomechanics

A symptom with many causes

Diarrhea that lasts beyond about four weeks is a sign to investigate, not a diagnosis. The workup usually separates inflammatory causes, malabsorption, infection, medication effects and functional causes such as IBS.

Malabsorption
When fat, lactose or other sugars are not absorbed, they draw water into the bowel. Celiac disease and lactose intolerance are common examples.
Medications
Antibiotics, metformin, magnesium, some antidepressants and many other medicines can loosen stools. Never stop one without your prescriber.
Hydration
Long-running diarrhea can drain fluid and salts. Dizziness on standing, very dark urine or confusion needs medical care.

The classical lensChannels, patterns, the five phases

Transformation that fails

The classics name this group xiè xiè, 泄瀉: food and fluid passing through before the Spleen can transform them. The Spleen dislikes damp, so most patterns involve damp, joined by cold, heat, stress or depletion.

Raising the clear
The Spleen lifts what is clear and the Stomach sends down what is turbid. When the Spleen's qi sinks, the clear falls with it, which is why some formulas for loose stools also lift qi.
Clear before you build
When damp-heat is active, it is cleared first. Tonifying a Spleen that is still full of hot damp tends to make things worse.

Patterns we often see

  • Spleen qi deficiency pí qì xū 脾氣虛
  • Spleen and Kidney yang deficiency pí shèn yáng xū 脾腎陽虛
  • Damp-heat in the intestines dà cháng shī rè 大腸濕熱

Patterns are examples of how this is often read, never a diagnosis. Yours is read at your visit.

How we work: one plan

A classical formula usually leads, with acupuncture and Master Tung's points in support, moxibustion over the belly or low back for cold patterns, and food advice for your pattern. The formula is revised as stools change. Your physician's workup comes first and stays part of the plan.

What chronic diarrhea is

Loose or frequent stools that last more than about four weeks are called chronic. The list of causes is long: inflammatory bowel disease, celiac disease, IBS, infections that linger, medication side effects, an overactive thyroid and more. That is why the first step is your physician's workup. Many people finish it with no clear cause, or with a label such as IBS-D, and the loose stools carry on.

What the episodes have in common

Patients often describe their diarrhea as random. Kept in a short diary, it rarely is. Watery stools that follow iced drinks or a rainy week suggest cold and damp. Soft stools with tiredness, worse when meals are skipped, point to a weak Spleen. A hot, urgent, foul stool reads as damp-heat. A trip to the bathroom just before dawn, with cold knees and a sore back, is the daybreak diarrhea the classics tied to the Kidney. The diary becomes part of the diagnosis.

How we work with it

Herbs usually lead. For a tired center with damp, a practitioner might consider Shen Ling Bai Zhu San (Ginseng, Poria and White Atractylodes Powder, 參苓白朮散); for daybreak diarrhea from a cold Kidney, Si Shen Wan (Four-Miracle Pill, 四神丸) instead. Both show how the pattern changes the approach, never a fixed recipe, and the formula changes as your stools do.

Acupuncture and Master Tung's points support the formula, with moxibustion over the belly or low back in cold patterns and food advice that favors warm, cooked meals where the pattern calls for it. We follow stool form, frequency and the timing of episodes with you.

Formulas here illustrate how pattern diagnosis shapes treatment. They are not recommendations; a practitioner selects and adjusts formulas for the individual.

Questions people ask

Why does my diarrhea keep coming back?

Once infection and other medical causes have been ruled out, the classical reading looks at what the episodes have in common. Loose stools after cold food, after stress, or before dawn each point to a different pattern, and the herbs follow that difference.

Can herbs help with loose stools?

Classical formulas for loose stools are some of the oldest in the herbal tradition. One is chosen for your pattern, checked against your medications, and adjusted as your stools change.

What about my child's diarrhea?

A child with diarrhea who is listless, has a dry mouth, cries without tears, has far fewer wet diapers or has blood in the stool needs same-day medical care. Bring the child to us only once a physician has checked them, and tell us what they found.

When is diarrhea an emergency?

Get medical care promptly for blood or black stools, high fever, signs of dehydration such as dizziness and very little urine, severe belly pain, or diarrhea after recent travel or a course of antibiotics.
