Floaters and vitreous detachment

For people with long-standing floaters that their eye doctor has checked and called stable. New floaters with flashes or a shadow are a different matter, and they come first on this page.

Two lenses, one plan

Wood 木 is the phase it is most often read through. Here the Western lens leads, with the classical lens alongside.

Gel, fibers and the retina behind them

The vitreous is mostly water held in a fine scaffold of collagen. As that scaffold breaks down, pockets of liquid form and fibers bundle together, and the shadows they cast are what you see as floaters.

Why a fresh PVD is checked
As the gel separates it can pull hard where it is stuck to the retina. That traction produces flashes and, occasionally, a tear, so a new PVD is examined, sometimes more than once.
Floaters or fog
Floaters move with the eye. A haze that stays put, or a blind patch, points to something else and needs its own exam.
Getting used to them
The brain often learns to ignore a stable floater. Fatigue, dry eyes and stress tend to make floaters feel more intrusive, and that layer is part of what supportive care works with.

The classical lensChannels, patterns, the five phases

Clouds and mist before the eye

The classical name 雲霧移睛 describes shapes drifting like mist across the visual field. The vitreous belongs to the Water Wheel, nourished by Liver blood and Kidney yin, and when those run low the inner water is thought to lose its clarity.

Turbid or thin
Floaters with heaviness, a coated tongue and sluggish digestion read as damp turbidity rising; floaters with dry eyes, poor sleep and a pale tongue read as blood or yin running thin. The herbs go in opposite directions.

Patterns we often see

  • Liver blood deficiency gān xuè bù zú 肝血不足
  • Liver and Kidney yin deficiency gān shèn yīn xū 肝腎陰虛
  • Phlegm and damp turbidity rising tán shī shàng fàn 痰濕上泛

Patterns are examples of how this is often read, never a diagnosis. Yours is read at your visit.

How we work: one plan

For floaters your eye doctor has checked, M48 micro acupuncture is combined with scalp acupuncture, Master Tung's points and electro-stimulation when appropriate, with a formula for the pattern and food advice from the same reading. Your ophthalmologist stays your primary eye doctor, and new flashes or a shadow go to them the same day.

Same-day eye exam if this is new

Most floaters are a nuisance rather than a danger. Some are a warning. See an eye doctor the same day, or go to an emergency department, if you have:

  • a sudden shower of new floaters, or one large new one
  • flashes of light, like a camera flash at the side of your vision
  • a curtain or shadow over part of your sight
  • floaters with a sudden blur or drop in vision

These can come with a retinal tear or detachment, which needs a dilated exam and sometimes urgent laser or surgery. Supportive care waits until the retina has been checked and found intact.

What floaters and PVD are

The inside of the eye is filled with a clear gel, the vitreous. With age it slowly turns more liquid, and fibers inside it clump. Those clumps cast shadows on the retina: dots, threads, cobwebs or rings that drift when you move your eyes and show up against a bright sky or a white screen.

At some point the gel usually pulls away from the back of the eye. That is a posterior vitreous detachment (PVD), a normal age change that often brings a burst of floaters and flashes. Most settle, but a minority tug the retina hard enough to tear it, which is why the first exam matters. Nearsighted people and those who have had eye surgery tend to meet this earlier. Conventional care is mostly watchful, with laser or vitrectomy kept for floaters that seriously limit sight.

How we work with it

For stable floaters we combine M48 micro acupuncture with scalp acupuncture, Master Tung's points and electro-stimulation when appropriate, chosen from your pattern. When thin blood leads, a practitioner might consider a formula such as Ming Mu Di Huang Wan (Bright Eyes Rehmannia Pill). When damp and turbidity lead, the herbs clear first. Food advice follows the same reading, see dietary therapy.

We keep track with you of how noticeable the floaters are in your day and how tired your eyes feel. Your eye doctor's dilated exams stay on schedule, and any new warning sign goes straight back to them.

Formulas here illustrate how pattern diagnosis shapes treatment. They are not recommendations; a practitioner selects and adjusts formulas for the individual.

Questions people ask

When are floaters an emergency?

A sudden shower of new floaters, flashes of light, a curtain or shadow over part of your sight, or a sudden blur all need an eye doctor or emergency department the same day. They can come with a retinal tear or detachment.

Can acupuncture make floaters go away?

We do not claim to remove floaters. The aim is supportive, working with eye fatigue, dryness and the pattern underneath, and many people find stable floaters bother them less over time.

Do I need an eye exam before I come?

Yes. Floaters should be checked with a dilated exam first, so we know the retina is intact. Bring the report if you have it.
