Macular edema

For people with swelling at the macula, whatever is causing it, who are already under a retinal specialist's care. Injections, drops and laser stay first; supportive care fits around them.

Why fluid collects at the macula

The retina is kept dry by tight seals in its vessel walls and by a pump in the support layer beneath it. When those seals loosen, from high blood sugar, a blocked vein or inflammation, fluid leaks faster than it can be cleared and pools at the macula.

Cystoid spaces
On OCT the fluid often forms small cysts within the retina, which is where the name cystoid macular edema comes from.
Treating cause and swelling
Anti-growth-factor injections, steroid injections or implants and anti-inflammatory drops treat the swelling, while the cause, such as diabetes or uveitis, is treated alongside.

The classical lensChannels, patterns, the five phases

The Spleen and the water roads

The Spleen transforms and moves fluids, and the Kidney governs water as a whole. When either weakens, fluid settles where it should not; at the macula that is read as damp gathering at the center of the eye.

Heat and stasis
Diabetic and inflammatory edema often adds heat scorching the fine vessels; edema after a vein occlusion adds blood stasis that holds water in place.
Formulas follow the pattern
Where Spleen damp leads, a practitioner might consider Ginseng, Poria and White Atractylodes (Shen Ling Bai Zhu San, 參苓白朮散); where heat or stasis drives the swelling, a different formula instead.

Patterns we often see

  • Spleen qi deficiency with dampness pí xū shī shèng 脾虛濕盛
  • Spleen and Kidney deficiency pí shèn liǎng xū 脾腎兩虛
  • Blood stasis with water retention xuè yū shuǐ tíng 血瘀水停

Patterns are examples of how this is often read, never a diagnosis. Yours is read at your visit.

How we work: one plan

Care combines M48 micro acupuncture with scalp acupuncture, Master Tung's points and electro-stimulation when appropriate, plus a classical formula reviewed against your injections and drops. Your retinal specialist stays your primary eye doctor, and visits are planned around injection dates.

Formulas here illustrate how pattern diagnosis shapes treatment. They are not recommendations; a practitioner selects and adjusts formulas for the individual.

What macular edema is

Macular edema is swelling of the macula from fluid that leaks out of retinal vessels. It is a result rather than a disease of its own: the usual causes are diabetes, a retinal vein occlusion, uveitis and the inflammation that can follow cataract or other eye surgery (cystoid macular edema). Central vision blurs, lines look wavy and colors look washed out. An OCT scan shows the thickening clearly.

Conventional care treats both the swelling and its cause. Depending on the cause that may mean injections that block vessel growth, steroid injections or implants, anti-inflammatory drops or laser, plus control of blood sugar or inflammation.

The cause shapes the plan

Edema after a vein occlusion, edema from diabetes and edema after surgery are read differently even though the scan looks similar. Diabetic and inflammatory cases often bring heat in the fine vessels into the reading, while swelling after a vascular event points more to blood stasis holding water in place. The cause also decides whom we coordinate with: your retinal specialist always, and your diabetes or rheumatology team where they are involved.

How we work with it

Edema care combines M48 micro acupuncture for the macula with scalp acupuncture over the visual cortex, distal Master Tung's points and electro-stimulation when appropriate. A classical formula that strengthens the Spleen and moves fluid is common, adjusted for the Kidney, heat or stasis behind it.

We follow your OCT thickness and acuity results between injections, and what you notice when reading.

Questions people ask

Can acupuncture reduce swelling in the macula?

We cannot promise that, and research on acupuncture for macular edema is limited. It is offered here as supportive care, never in place of injections, drops or laser. Your OCT scans are how any change is measured.

Will herbs interfere with my eye injections?

Every formula is reviewed against your injections, eye drops and other medicines before it is prescribed, and visits are planned around your injection dates.

Why do you ask about my digestion for an eye problem?

In classical terms fluid that gathers where it should not is often read through the Spleen and Stomach, so appetite, bloating, energy after meals and the tongue all help choose the points and formula.
