Low back pain

For lower back pain that keeps flaring, a back that locks up after a bend, or an ache that never quite leaves. We look for what is loading the back, treat the tissue and change the movement, alongside your physician.

Two lenses, one plan

Water 水 is the phase it is most often read through. Here the Western lens leads, with the classical lens alongside.

The Western lensAnatomy, physiology, biomechanics

Discs, joints and the muscles that brace them

Most low back pain is mechanical: a strained muscle, an irritated facet or sacroiliac joint, a disc, or a mix of these. The deep stabilizers of the lumbar spine, such as the multifidus, are thought to switch off after an episode, which may leave the back more open to the next one.

Spasm as protection
A back that locks up after a bend is usually muscle bracing a sore segment. The quadratus lumborum and the long paraspinals can keep up that brace well after the original strain has settled.
The hip and the upper back
A stiff hip or an upper back that will not rotate pushes movement down into the lumbar spine, which is built more for stability than for twisting.
The sacroiliac joint
Where the spine meets the pelvis, the SI joint can refer pain into the buttock and the back of the thigh, and it is easily mistaken for a disc.

The classical lensChannels, patterns, the five phases

The Bladder channel and the Governing vessel

The Bladder channel runs in two lines down each side of the spine, and the Governing vessel, 督脈, runs up its center. A fresh strain is read as stasis in these channels; back pain that keeps returning and worsens with fatigue is read through the Kidney beneath them.

Weizhong, behind the knee
An old point song says: for the back, seek Weizhong (委中, BL-40). It is a classic distal point for the low back, used especially when a strain has left stasis behind.
Lingu and Dabai
Two of Master Tung's points on the hand, 靈骨 and 大白, are often needled on the opposite side while you gently move your back, so a guarded area does not have to be needled first.

Patterns we often see

  • Cold and damp obstruction of the low back hán shī yāo tòng 寒濕腰痛
  • Blood stasis after a strain yū xuè yāo tòng 瘀血腰痛
  • Kidney deficiency low back pain shèn xū yāo tòng 腎虛腰痛

Patterns are examples of how this is often read, never a diagnosis. Yours is read at your visit.

How we work: one plan

A visit usually opens with a movement screen of the hips, pelvis and upper back. Then dry needling and orthopedic acupuncture of the lumbar and hip muscles found on assessment, Active Release Techniques where fascia has stuck, and functional movement corrections for the hinge and the hips. Classical and Master Tung's points cover the pattern and the very sore days. Your physician stays in the loop for anything that needs a medical workup.

What low back pain is

Low back pain covers everything from a one-off spasm after lifting the groceries to an ache that has come and gone for years. Most of it is mechanical, meaning it comes from how muscles, joints and discs are loaded rather than from disease. Your physician rules out the rest, and is the right first call for back pain that is new and severe.

When to get urgent care

Get emergency care first, or call 911, if back pain comes with:

  • loss of bladder or bowel control, or numbness in the groin and inner thighs (the saddle area)
  • weakness that is getting worse in a leg, or a foot that slaps or drags
  • severe, sudden pain with no clear cause, or pain after a fall or accident
  • fever, unexplained weight loss, or a history of cancer

Pain and dysfunction are rarely at the same address

A low back that hurts is often a low back doing someone else's job. Hips that no longer rotate, an upper back that has stiffened at the desk, or glutes that have gone quiet all push extra movement into the lumbar segments. So we look up and down the chain as well as at the sore spot.

How we work with it

The muscles found on assessment, often the multifidus, quadratus lumborum, the glutes or the deep hip rotators, are treated with dry needling and orthopedic acupuncture. Active Release Techniques free fascia that has stuck, and functional movement corrections retrain the hinge, the squat and the way you get out of a chair. If the leg is involved, see sciatica and bulging discs.

What we track together: how long you can sit, bend or walk, how you sleep, and how the movement that hurt most now feels.

Questions people ask

Can acupuncture help lower back pain?

Acupuncture for low back pain has been widely studied, and here it is combined with hands-on work and movement corrections rather than used alone. Whether it fits your case depends on what the evaluation finds is driving the pain.

My back just went into spasm. Should I rest or keep moving?

For most simple spasms, gentle movement within comfort tends to help more than bed rest. Distal points can be needled while you move, which is often easier than needling a back that is locked. If any of the urgent signs on this page are present, see a physician first.

Will I be sore after dry needling of the back?

Some soreness for a day or two is common, a bit like after a hard workout. Pain that clearly worsens beyond that, or any new numbness or weakness, is worth telling us about promptly.

Do you treat sacroiliac joint pain?

Yes. The SI joint is checked at every low back evaluation, since it can refer pain into the buttock and thigh and is easily confused with a disc.
