Macular Edema — What It Is

Macular edema occurs when fluid accumulates in the macula — the central, high-resolution region of the retina responsible for sharp central vision. It is not a disease in itself but a response to underlying pathology: diabetic retinopathy, retinal vein occlusion, uveitis, and post-surgical inflammation are common causes. The fluid thickens the macula and distorts central vision, producing blurring, wavy lines, and washed-out colors.

Conventional management is typically active — anti-VEGF injections (bevacizumab, ranibizumab, aflibercept), intravitreal corticosteroids, or laser photocoagulation — depending on the underlying cause. These are the primary treatments and this program does not replace them.

How TCM Reads Macular Edema

Classical Chinese medicine reads macular edema through the convergence of two Five-Wheel territories. The macula sits in the Water Wheel (水輪) — Kidney territory — while the fluid metabolism dysfunction points to the Flesh Wheel (肉輪) — Spleen territory. The dominant pattern is:

  • Spleen Qi deficiency with dampness and fluid accumulation. The Spleen governs the body’s fluid transformation and transportation. When Spleen Qi weakens — through diet, chronic illness, or constitutional tendency — fluids accumulate rather than circulate. In the eye, this manifests as the edematous thickening the macula. Signs: fatigue worse after meals, bloating, soft stool, foggy vision with heaviness, tongue with teethmarks and a thick coating.
  • Kidney-Spleen dual deficiency (more complex presentations). When the underlying driver is diabetic or ischemic, a combined Spleen-Kidney pattern is often present — the Kidney’s fluid-governance and the Spleen’s transportation both compromised simultaneously.

Herbal prescription: Spleen-damp presentations draw from Bu Zhong Yi Qi Tang (補中益氣湯) or Shen Ling Bai Zhu San (參苓白朮散) to strengthen Spleen Qi and transform dampness. When the Water Wheel Kidney-Yin layer is also involved, a combined formula addresses both axes. What to Expect

This program helps manage symptoms commonly associated with macular edema — including central visual clarity patterns and fluid-related visual distortion. It does not replace anti-VEGF therapy, corticosteroid injections, or ophthalmologic monitoring. Patients should maintain their scheduled retinal specialist appointments throughout any course of care with us. Individual results vary.
