Central Serous Retinopathy — What It Is
Central serous retinopathy (CSR, also called central serous chorioretinopathy) occurs when fluid leaks from the choroid — the vascular layer beneath the retina — and collects under the macula, causing a localized detachment of retinal tissue. Patients typically notice a blurred or distorted patch in central vision, sometimes with objects appearing smaller, dimmer, or curved. CSR is strongly associated with elevated cortisol — it is more common under high physical or psychological stress, and has a well-documented link to corticosteroid use.
Many cases resolve on their own within a few months as the fluid reabsorbs. Some become chronic or recurrent. Your ophthalmologist may recommend observation, laser treatment, or other intervention depending on duration and severity — this program is a complement to that care, not a substitute for it.
How TCM Reads Central Serous Retinopathy
Classically, the macula sits within the Flesh Wheel (肉輪), governed by the Spleen, but CSR’s strong tie to stress and cortisol points Chinese medicine toward a different root: the interaction between the Liver and the Heart-Kidney axis.
- Liver Qi stagnation. The pattern most directly tied to CSR’s stress-trigger. Chronic emotional pressure, unresolved frustration, or sustained high-demand states cause Liver Qi to bind rather than flow — and Liver-channel pathology is read as disrupting the smooth flow of fluids to the eye, including at the macula. Signs: onset or flare during a period of significant life stress, tension, irritability, a sense of things being “stuck.”
- Heart-Kidney discoordination (心腎不交). When the normal downward-cooling of Heart fire and upward-nourishing of Kidney water fall out of sync — often from prolonged stress or overwork — the resulting internal heat can drive fluid accumulation patterns. Signs: insomnia or restless sleep, anxiety, night sweats, a racing quality to the mind alongside the visual symptoms.

What to Expect

Continued ophthalmologic monitoring is essential, particularly to track fluid resolution and rule out recurrence or chronic changes that may need laser or other intervention.
