Diabetic Retinopathy — What It Is

Diabetic retinopathy is damage to the retina’s blood vessels caused by prolonged high blood sugar. In its earlier (non-proliferative) stage, vessels weaken and leak, sometimes causing swelling at the macula (diabetic macular edema). In its advanced (proliferative) stage, the retina grows fragile new blood vessels in response to poor circulation — vessels prone to bleeding and scarring that can lead to serious vision loss if untreated.

Blood sugar control is the single most important factor in preventing progression. Ophthalmologic monitoring (dilated eye exams at least annually for most diabetic patients) and, when indicated, laser treatment or anti-VEGF injections are the standard of care for active disease.

How TCM Reads Diabetic Retinopathy

Diabetic retinopathy is understood as a downstream complication of the pattern classical Chinese medicine calls Xiao Ke (消渴, “Wasting-Thirst”) — the traditional framework for diabetes itself.

  • Yin deficiency with internal heat. The core Xiao Ke pattern — deficient Yin fails to control internal heat, and that heat, sustained over years, is read as scorching and drying the body’s fine vessels and collaterals (絡脈), including those feeding the retina. Signs: thirst, dry mouth, heat signs, the long-standing systemic picture typical of diabetes.
  • Qi and Yin deficiency with Blood stasis in the collaterals. In more advanced or longer-duration cases, Qi deficiency compounds the picture — Qi too weak to move Blood properly — and the resulting stasis in the retina’s fine vessels is read as underlying the leakage, hemorrhage, and abnormal vessel growth seen in more advanced disease. Signs: fatigue alongside the Yin-deficiency heat signs, longer diabetes duration, more advanced retinal findings.

What to Expect

This program supports the constitutional pattern associated with diabetic retinopathy — it does not repair vascular damage that has already occurred and is not a substitute for blood sugar control. Continued ophthalmologic monitoring, including dilated eye exams, and any indicated laser or injection treatment remain essential.
