Posterior Vitreous Detachment — What It Is

Posterior vitreous detachment (PVD) occurs when the vitreous humor — the gel filling the back of the eye — separates from the retinal surface. This is a normal age-related change that typically begins in the 50s to 70s, often causing a sudden onset of floaters and flashes of light as the vitreous peels away. In most cases, PVD resolves without complications as the floaters settle and the flashes subside.

However, a small percentage of PVD cases involve vitreous traction on the retina that can tear or detach the retina — a surgical emergency. Anyone experiencing new sudden floaters or flashes should have an urgent ophthalmologic evaluation to confirm the retina is intact before any supportive program begins.

How TCM Reads PVD

PVD presents within the Water Wheel (水輪) — the vitreous and inner eye territory of the Kidney. Classical Chinese medicine reads the vitreous detachment event and its aftermath through:

  • Kidney and Liver Yin deficiency with vitreous desiccation. The vitreous gel requires ongoing nourishment from Kidney Yin and Liver Blood. As these diminish with age, the vitreous loses its fluid substrate and begins to liquefy and collapse — the physiological substrate of PVD. The classical picture: advancing age, tinnitus, low-back ache, dry eyes, diminished night vision all preceding the PVD event.
  • Qi and Blood stasis (post-event). After the vitreous detaches, residual floaters can reflect localized Blood stasis in the ocular channels — the debris shadows are read in TCM terms as congestion in the clear-orifice pathways.
